Annual Congregation Report Form: This form is completed by member congregations, at the end of each fiscal year, to report their actual income and expenses.
Membership List Update: All member congregations are required to submit an updated membership list with their completed Annual Congregational Report Form. The List Maintenance Update Form should be completed and returned with the updated membership list.
